National League for Nursing (NLN) PAX Examinations
- Required for admission into the ADN and PN programs
- Registration is required in advance of the desired test date

Student Health Statement / Medical Release
Students accepted into any of the following College of Health Sciences programs will be required to provide documentation of good physical and mental health by completing the Student Health Statement/Medical Release Form. Documentation of all health problems and a medical release is required. Copies of immunization records and valid CPR certification must be submitted to the College of Health Sciences. No student will be allowed to attend a clinical rotation without the required health data records on file in the program director's office.

Estimated cost for the 2009-2010 school year - costs may change without notice and additional costs may be incurred throughout a program's entirety.
Students are classified as in-state if they have lived in Arkansas, Oklahoma, Missouri, Tennessee, Mississippi, Louisiana, or Texas for at least six consecutive months prior to the beginning of the term for reasons other than attendance at UA Fort Smith.
